Dr. Heidi Wittenberg 是一位在旧金山的妇产科出身的 SRS 医生。她的诊所是MoZaic Care，另外一位医生是她的学生Dr. Adam Bonnington。她是美国西海岸为数不多的可以进行腹膜手术的医生。现时（2022 年 2 月），她进行手术的医院是 San Francis Memorial Hospital。

特点 ¶

由于 Wittenberg 是妇产科出身，所以她对阴道及外阴的解剖结构会比一些整形外科出身的医生好。由她进行的 SRS 的解剖结构以及术后敏感度会比其他医生好一些。但是同样由于她并不是整形外科出身，她对于外在美学的追求并没有其他医生高，所以外观并不一定会很好。

要求 ¶

• 关于推荐信：要求至少有两封精神科医生的信和一封 HRT 医生的信，其中一封的精神科医生必须有博士或等同学位（推荐信内容的具体要求在页面底部）
• BMI 要求：强制要求低于 35 且高于 20，但是在正常范围内为最佳（因为伤口缝合原因）
• 年龄要求：18 岁及以上无需家长同意

流程 ¶

1. 在官网 Contact 处预约 Initial Consultation
2. 参加 Initial Consultation（可以选择和 Wittenberg 或者和她的学生Dr. Adam Bonnington 进行 Initial Consultation。和她的话需要等 4-5 个周；和她学生的话等大约 2-3 周即可）
3. 在预约手术前需准备好3封推荐信并提交给诊所审核（推荐信不齐无法预约手术）
4. 获得手术日期（手术只会在周一/周二），可以选择加入 Cancellation list（如果前面有人取消/延后有机会提前手术时间。注意： Cancellation list 时间非常不确定，一定要做好心理准备即使有更近一些的位置也不一定合适。）
5. 在临近手术日期时处理好保险问题（pre-authorization）
6. 术前1月要进行抽血化验、术前体检（clearance）、EKG等，MoZaic会出具具体的 lab order 可以直接拿给PCP或外面的其他lab。
7. 手术前1周内进行术前咨询（大概率为周三周四），并在医院checkin。
8. 手术时间大约 5 小时（腹膜）或4小时（皮瓣），手术当天需要自己在6:30到医院，术后第 3 天后出院。
9. 术后一周进行第一次 post-op 拔尿管并开始通膜。要求在SF市内停留到第二次 post-op （约两周），在 bay area 停留到第三次 post-op （约三周）

术后通膜要求 ¶

• 前三个月：每天3次，每次10-30分钟
• 3-6个月：每天1次，每次10-30分钟
• 6-9个月：每周3-4次，每次10-30分钟
• 9个月及以后：每周1-2次，每次10-30分钟

保险 ¶

现时（2022 年 2 月），Wittenberg 接受的网络内保险有：

• Blue Cross/Blue Shield PPO（除 Covered CA Plans 和 Pathway PPO）
  • UC Ship (2024年5月，据接待人员描述，UCDavis的保险出现了报销问题，请注意。)
• San Francisco Health Plan
• OHIP and British Columbia（加拿大，不能报销腹膜 SRS 费用）

• The patient’s general identifying characteristics.
• The initial and evolving gender, sexual and other psychiatric diagnoses.
• The duration of their professional relationship, including the type of psychotherapy or evaluation that the patient underwent.
• The eligibility criteria that have been met and the mental health professional’s rationale for hormone therapy or surgery.
• The degree to which the patient has followed the Standards of Care to date and the likelihood of future compliance.
• Whether the author of the report is part of a gender team.
• That the sender welcomes a phone call to verify the fact that the mental health professional actually wrote the letter as described.
