醋酸環丙孕酮（Cyproterone Acetate，俗稱“色普龍/色譜龍”）是一種常用的抗雄藥物，多用於男性前列腺癌的治療、女性因高雄激素引起的青春痘、脫髮等症狀治療1 2；現為 GAHT 中使用最多的抗雄藥物之一3。常見的商品名有 Androcur®（安得卡），Siterone®等。

該藥具有極強的孕激素作用，抑制卵巢功能4，故也用於複方避孕藥，如“黛麗安”（Diane-35）。

  • 「日色」即醋酸氯地孕酮（Chlormadinone Acetate），雖同樣具有抗雄激素活性5，但並非醋酸環丙孕酮，請不要混淆

使用方式與用量

  • 服用方式：口服（無明顯味道）
  • 建議使用劑量：5 ~ 12.5 mg/天5
    • 多項研究發現 10 mg/天左右劑量已可最大程度抑制睪酮，且效果與高劑量一致6。進一步研究則發現，每週兩次 12.5 mg（等效約 3.5 mg/天）的超低劑量同樣可以實現睪酮水平的充分抑制7。最新發布的《跨性別和多元性別人群健康照護指南》第八版（SOC 8）亦建議 10 mg/天的劑量3。
    • 超出建議劑量會有更大的副作用與健康風險（見下）；低於建議劑量可能會使睪固酮抑制效果不充分。
    • 可用切藥器切為四片（即每份 12.5 mg），每 1 ~ 3 日一服。
  • 達到效果的情況下盡量減少用量。
  • 建議適量補充維他命 B12。
  • 停藥時須逐步減量。

副作用

  • 腦膜瘤風險：色普龍可能誘發腦膜瘤，風險與劑量和使用時長正相關。建議儘可能以更小劑量服用；如累積劑量已很大（高於 10 g），建議進行核磁共振掃描（MRI）8。
  • 憂鬱：色普龍可能導致憂鬱風險升高。主要原因是色普龍對於生理男性起到了抗雄激素和潛在的糖皮質激素作用，並可能誘發維他命 B12 缺乏9，而維他命 B12 的缺乏和多種精神/情緒狀況的改變有關，包括憂鬱10。有必要適量補充維他命 B12。
  • 高血壓與高血糖：色普龍可能會提高體內胰島素抗性，從而引起血糖偏高11。糖尿病患者應慎用，用藥時抗糖尿病製劑或胰島素的需求可能改變；併發血管病變時停用2。
  • 血栓與乳腺癌風險：與動情素合用時可一定程度提高其風險12 13。此外，也有色普龍單藥引起更高的血栓風險之報告14。
  • 高催乳素水平：色譜龍可明顯增加催乳素水平，且隨劑量上升，嚴重時可引起垂體催乳素瘤。應定期檢查催乳素水平6。
  • 肝毒性：色普龍最嚴重的副作用，與劑量相關，表現為 AST、ALT 等水平升高；嚴重時可引起肝衰竭甚至死亡6 1 2。使用前應當檢查肝生化值，使用時定期檢查1。
  • 暫未明確 過早引入色普龍（尤其高劑量）可能造成乳房發育不良15。
